AN EVALUATION OF A NURSE-LED MULTIDISCIPLINARY PATIENT CENTRED DIABETES SELF-MANAGEMENT EDUCATION PROGRAMME IN NIGERIA

Abstract

Background: Diabetes is on the increase globally, especially in African countries. Nigeria in particular has a high prevalence of diabetes type 2. There is evidence that improved diabetes outcomes are related to self-management and improved health education.

Purpose: The purpose of the study was to determine whether a nurse-led multidisciplinary patient centred diabetes self-management education programme would improve the management of key diabetes outcome measures.

Setting. Diabetes outpatient clinics in tertiary and secondary hospitals in Nigeria.

Study design: Quasi-experimental, two group, before-and-after study.

Methods: Two groups of participants (n=28) were selected from alternate day clinic attendances, 15 for the intervention group and 13 for the control group. A nurse-led multidisciplinary patient centred diabetes self-management education programme was developed and implemented over 5 weeks in 2014. The control group was exposed to normal clinic procedures. Knowledge and outcome measures data for both intervention and control group were collected and compared.

Results: At baseline the intervention and control groups did not differ significantly with regard to knowledge and outcome measures. After the intervention the intervention group had significantly lower FBS (p = 0.01) and BMI scores (.025) than the control group. BMI did not significantly differ between the two groups, but FBS did (p = .012).

Conclusion: The introduction of a nurse-led education programme improved key diabetes outcome measures for patients attending diabetes outpatient clinics at two hospitals.

Recommendation: Self-management education programmes have the potential to improve diabetes outcomes but further studies need to be done to confirm this.
